Huntsville ISD Athletics recommends all student-athletes receive an annual sports physical.  We prefer May to May physicals because they ensure your student-athlete has the latest physical on file.   The form for your medical history and physical is ready for download at the bottom of the page.

• Every Huntsville ISD Athlete must have a current 2024-2025 Physical on file before participating in practice/sport.  What is current?   May 1, 2024 - April 2025 to participate in the coming academic year.
• Once a year, Huntsville ISD partners with Texpress Urgent Care and Houston Methodist for FREE physicals.  The FREE physical is open to incoming 6th - 11th grade students.  The 2024 date is MAY 13 at Huntsville HS.  MPMS and HIS students will be provided with transportation
• Coaches will hand out the instruction/information packets for the FREE physical.  Please make sure you review and complete forms with signatures and bring them on the day of the physical
• * You may choose to use your physician for your student-athlete physical.   Please use the form in the link below to complete and return to your athlete's coach/sport.

Per Texpress, we encourage students/parents to leave blank any part of the form they might answer yes on.  This assists the provider to discuss any positives and have them documented appropriately as well as keeping the students/parent from having to redo the form.  Anyone with a known condition that has previously had to be cleared by the cardiologist (for example) will need to bring their most recent specialist notes with them to the clinic.
